Crafter Registration

$50 non-refundable entry fee plus 23% commission. $10 additional table fee per show (does not include extra space). $10 late fee. Entry fees must accompany this registration form. Applications will not be accepted without photos of display booth. All shows are "central-checkout". Vendors DO NOT "sit and sell". If you register for multiple shows, submit separate checks per show. $35 charge on returned entry fee checks. Make all checks payable to: Woods and Amann, LLC. Mail to: Cameron Amann, 21217 107th Ave SE, Snohomish, WA 98296. WASHINGTON STATE SALES TAX: All Washington State Sales Tax will be claimed and reported by Woods & Amann, LLC. EXHIBITOR CHECKS: Will be mailed within 14 business days following the show. A Misc 1099 will be issued for earnings of $600 or more. LIABILITY LIMITATIONS: Woods and Amann, LLC will not be responsible for losses due to the following: Merchandise or any display items that may be lost, stolen or damaged in any way * Personal injury to the exhibitor during set-up, pick-up or during show proceedings * destroyed by fire, weather, water, faulty buildings or other * Each vendor is liable for any personal injury that may occur from his own crafts and/or display pieces during set-up, show and tear-down. I have read the exhibitor agreement and understand THE STARVING HOUSEWIVES CRAFT SHOWS and WOODS AND AMANN, LLC will not be held responsible for losses due to the following: Merchandise or display items that may be lost, stolen or damaged in any way, any or all items destroyed by fire, weather, water or other causes, personal injury that may result during set-up, tear-down or during show proceedings and in addition hold Woods and Amann, LLC harmless from any personal or indirect injuries that may result from my crafts and/or display pieces at all times. I will comply with necessary county and state regulations while participating in the craft show(s).
