$50.00 nonrefundable entry fee plus 23% commission. $10 additional for an 8' table for the October show only * Entry fees must accompany this registration form * Applications will not be accepted without photos of display booth * All shows are "central-checkout" only. Vendors DO NOT "sit and sell" * Separate checks per show * $25.00 charge on all returned entry fee checks * Make all checks payable to: Woods and Amann, LLC * Mail to: Cameron Amann, 21217 107th Ave SE, Snohomish, WA 98296.
SET-UP SCHEDULE: Always one day before show 1 pm to 5 pm. Exceptions apply. Booth space may be limited after 5 pm. PICTURES: New vendors submit three or more pictures of crafts AND booth display with this registration form. VENDOR NUMBER: If you do not currently have a "vendor" number one will be issued to you on your confirmation postcard. CONFIRMATION: Will be sent approximately two weeks before show and will confirm your vendor number, set-up time and volunteer work schedule. If show is full your entry fee and pictures will be returned as quickly as possible. WORK SCHEDULE: Please indicate your preferred four hour work shift. PRICE TAGS: Any color ok * All merchandise must be clearly marked with price AND your vendor number. String or sticky tags ok. DISPLAY BOOTH: You are responsible for complete booth displays including tables, clip lights (no bulbs over 60 watts), ext cords, back-drops, etc. All table coverings must be floor length and light in color. EXHIBITOR CHECKS: Will be mailed within 14 business days following the show. All Washington State Sales Tax will be claimed by Woods and Amann, LLC and reported. Misc 1099 will be issued for earnings of $600 or more. LIABILITY LIMITATIONS: Woods and Amann, LLC will not be responsible for losses due to the following: Merchandise or any display items that may be lost, stolen or damaged in any way * Personal injury to the exhibitor during set-up, pick-up or during show proceedings * destroyed by fire, weather, water, faulty buildings or other * Each vendor is liable for any personal injury that may occur from his own crafts and/or display pieces during set-up, show and tear-down.I have read the exhibitor agreement and understand THE STARVING HOUSEWIVES AND WOODS AND AMANN, LLC will not be held responsible for losses due to the following: Merchandise or display items that may be lost, stolen or damaged in any way, any or all items destroyed by fire, weather, water or other causes, personal injury that may result during set-up, tear-down or during show proceedings and in addition hold Woods and Amann, LLC harmless from any personal or indirect injuries that may result from my crafts and/or display pieces at all times.
